AGREEMENT WITH THE GENERAL TERMS OF USE OF THE 3DERM ONLINE POLYCLINIC SERVICE

• Polyclinic 3DERM provides video consultations by appointment through online polyclinics, in order to provide patients with greater access to dermatological care. The service is designed to provide a consultation service to those patients who are unable to see a doctor in person, or who need such a consultation that does not require personal contact with the doctor. 
• The online polyclinic is not intended to be a complete healthcare service or an alternative to a real clinic visit, procedure or surgery and there will be cases when advice provided via video consultation will result in an appointment and further live treatment.
• THE ONLINE POLYCLINIC IS NOT INTENDED FOR MEDICAL EMERGENCIES. Do not use it if you think your health problem is an emergency, and under no circumstances should you use it if you experience a medical emergency. The user agrees, confirms and accepts that he is aware of the fact that the Online Polyclinic is not intended for any type of medical emergency. In case of doubt and the need to take care of any urgent medical conditions, the polyclinic recommends that you call the competent emergency service. 
• 3DERM Online Polyclinic can be accessed through multiple websites, social networks, devices, and other means, whether owned and/or operated by us, including, without limitation, the website www.poliklinika3derm.eu

GENERAL TERMS OF USE

• These are the Terms of Service Provision and Use (hereinafter referred to as "Terms") for access and use by any person ("User", "Patient" or "You") of the 3DERM online polyclinic through which dermatological consultation services are provided.
• When the terms "we", "us", "our" or the like are used in these Terms, they refer to Polyclinic 3DERM, which owns and operates the Online Polyclinic.
• By accessing or using the online polyclinic or by clicking on a button or icon indicating that you have read and accepted the Terms of Service and Use, you understand and agree to these Terms. Please read these Terms carefully before you start using them.
• These conditions are subject to change. When these Terms change, the new version will be published on the website www.poliklinika3derm.eu.
• The user declares under his own responsibility that the information he provides is accurate, and is informed that the use of a false identity and the dissemination of false information may lead to civil and/or criminal prosecution.
  In order to avoid invalid reservations, Poliklinika 3DERM may use the provided mobile number and/or e-mail address to confirm the patient's request for an appointment.

USE OF THE ONLINE POLYCLINIC

• The 3DERM online polyclinic enables you to have a video consultation with the doctors of the 3DERM polyclinic (collectively referred to as the "doctor") for the purpose of conducting a dermatological consultation service, providing a second opinion, planning treatment, arranging treatment appointments, counseling, obtaining information, advice or any other contribution or services of the polyclinic.
• The service enables patients to find a doctor, to request an appointment from a doctor, to perform video-consultations, to receive confirmations of the appointment of video-consultations, at the same time providing the doctor with the possibility of remote consultation, monitoring and correction of the patient's therapy. Thus, the polyclinic can be used, among other things, to access and facilitate the availability of: (a) consultation, second opinion, treatment planning, arranging treatment appointments, counseling and information specifically and individually provided and delivered to you; and (b) general information and content that is publicly available and not transmitted personally to you.
  The video consultation provided by the polyclinic provides a secure one-on-one encrypted channel between the patient and the doctor. In providing this service, the polyclinic respects all the privacy rules prescribed by the provisions on the protection of personal data. The user has accepted all the provisions on the protection of personal data when creating his own profile, and is aware that he can read them on the publicly published link on personal data protection.
• Neither the patient nor the doctor is allowed to record, copy or broadcast any content or excerpt of the content of the video consultation. Both are informed that any violation of image rights, respect for privacy or professional and medical confidentiality may be subject to sanctions, including criminal sanctions.
  Equipment (smartphone, internet connection, etc.) that enables access and use of the 3DERM online polyclinic service is at the patient's own expense, as well as related electronic communication costs (especially telephone costs, Internet access costs). It is the patient's responsibility to obtain information about the price of using the previously mentioned equipment or services from the operator before using the service of the 3DERM online polyclinic.
• The patient is invited to provide the following conditions in advance during the consultation through the 3DERM online polyclinic: smartphone or computer, camera and microphone, internet connection on his computer allow video and audio permissions to our 3DERM online polyclinic service ensure a quiet and sufficiently lit environment
  If the environment, equipment or license conditions are not met, the doctor can stop the video consultation. In the event that the patient feels negatively during the video consultation and counseling under the influence of impaired performance or quality, he is obliged to inform us about it.
• The doctor who provides the service of the online polyclinic 3DERM is the only one who has the right to assess the suitability of the patient's remote consultation and can terminate the video consultation in case the conditions for remote consultation are not met.
• The patient understands and accepts that the 3DERM online polyclinic service may be charged by the 3DERM online polyclinic, whose responsibility is to inform the patient about this in a timely manner.
• The services of the 3DERM online polyclinic are not included in the billing system of the Croatian Institute for Health Insurance and cannot be obtained through a HZZO referral.

LIMITATION OF LIABILITY

• If you think you need emergency medical help, CALL A DOCTOR OR THE EMERGENCY NUMBER (112 or 194 IN CROATIA) IMMEDIATELY AND URGENTLY. Do not rely on electronic communication or communication through the 3DERM online clinic for urgent medical needs. This 3DERM online clinic is not designed to facilitate medical emergencies. YOU ACKNOWLEDGE, ACKNOWLEDGE AND AGREE THAT THE PLATFORM IS NOT INTENDED FOR USE IN ANY OF THE FOREGOING CASES AND THAT YOU MAY NOT USE THE PLATFORM IN ANY OF THE FOREGOING CASES.
• 3DERM does not guarantee that video consultation through the Online Polyclinic is the best approach to advice in the field of dermatological diseases or indeed the appropriate method of treatment for your particular health concern or health problem. Furthermore, Online Polyclinics is not a substitute for your doctor and primary care provider. You agree to contact your doctor immediately if your condition changes or your symptoms worsen. If you need emergency help, contact the nearest emergency service center immediately.
• Online polyclinic service, consultation or video consultation with doctors cannot and does not replace a visit to the clinic and a "face-to-face" consultation. We recommend and encourage you to check all the information you receive from the doctor of the Online polyclinic service provider. Any reliance on such information is entirely at your own risk and responsibility.
• Do not neglect, avoid or delay obtaining medical advice from your doctor or other qualified medical specialist, visited in person, because of information or advice you have received through the Online Polyclinic.
• Online polyclinic 3DERM will try to solve any error in the system as soon as possible. Online polyclinic 3DERM, as much as possible, to inform its users in advance of any availability restrictions due to maintenance activities and/or troubleshooting of the Online polyclinic 3DERM system.
• The user is informed that communication transmission systems, computer systems that provide hosting of web portals or doctors' computers may be susceptible to malfunctions, as well as any computer system. The user hereby waives all possibilities of settlement against the Online polyclinic 3DERM or affiliated entities due to a technical failure in the use of the provided services or due to the failure to establish a video consultation
• The user is informed that the Online Polyclinic 3DERM is not responsible for the breach of confidentiality of electronic messages or other data sent due to improper implementation by the user of security instructions related to their own system.
• The user accepts, confirms and agrees that the platform is made "as is", and will not have any claims or statements against us. Use of the platform is at your own risk.
